If your child requires oral medications for a prevalent medical condition that would ever be kept and administered at school, or an EpiPen for anaphylaxis, please download/print the appropriate files below and have them filled out completely by your physician. Forms must be completed in their entirety and signed by physicians and parents/guardians before being returned to school.
